OK, I've been trying to find out quite how these towball bicycle carriers work. In my mind it's a bit like a stabilising hitch gripping onto the ball itself. For these hitches "it is essential that all paint and grease is removed from the towball prior to use" (https://www.westfalia-automotive.com/uk/products/towbars/fixed-towbar/)

Have people cleaned and removed the paint from their towball to use a towball mounted bike rack?

Nope. Mine was the removable and never had a problem. It stays on all the time now.

My factory fitted didn't have the actual Ball painted.
 
If you use a trailer prior to the bike carrier, the paint will be gone without any effort.

Well, with a stabilising hitch it affects the function/friction coefficient. Wondered if it were the same with the bike carrier...
 
Grease removed before bike carrier fitment but only necessary if a trailer has been used.
Paint is thin and dry so no need to remove. If it was rust would set in which would need to be removed before fitting bike rack, repeatedly over time.
 
Our Westfailer tow bar was painted. No problem with the bike carrier - no paint left now.
